References

If you are not currently on staff at a YWAM base, please send one to a previous YWAM leader. The third reference can be a friend or colleague. We will not accept references from family members.

DTS Graduation Certificate

Please email staff.registrar@ywamtokyo.org or mail a copy of your DTS Graduation certificate for verification purposes.

Health Form

You will also need a health form, signed by your doctor. This will need to be emailed or mailed to us. The link to this document in found here: Health Form

Under 20's Form

If you will be under the age of 20 when you wish to start, a physical copy of the signature of parent or responsible party is required for the Release Liability Agreement and Consent for Treatment. Please print and have it signed before scanning and emailing staff.registrar@ywamtokyo.org to us.
